Chronic neurological diseases like multiple sclerosis are marked by persistent inflammation in the central nervous system (CNS), primarily driven by overactive microglia. These immune cells undergo metabolic shifts that impact their energy production and function, contributing to the sustained inflammation. Our study revealed that mitochondria, the energy powerhouses within microglia, become dysfunctional in chronic inflammation, leading to excessive production of harmful reactive oxygen species (ROS) that fuel inflammation and cause nerve damage. By blocking a specific part of the mitochondrial energy pathway (complex I), we successfully reduced brain inflammation and nerve damage in a multiple sclerosis animal model. This discovery suggests that targeting this energy pathway in microglia offers a potential new therapeutic avenue for treating chronic inflammatory CNS diseases.
Metabolic Control of Microglia 2024
Microglia, the resident innate immune cells in the brain, have their functions closely tied to their metabolism. This chapter explores the concept of immunometabolism, focusing on how microglial metabolic processes influence their diverse roles in the central nervous system (CNS). It examines the impact of metabolism on microglia during different CNS states, including healthy conditions, development, aging, and inflammation. In the context of CNS inflammation, the chapter discusses how metabolic changes in microglia can trigger, resolve, or sustain inflammatory responses. It also explores the potential of targeting microglial metabolism as a therapeutic approach for chronic CNS diseases with significant immune cell activation.

A groundbreaking study has just shown promising results in the treatment of progressive multiple sclerosis (MS). Fifteen patients with this debilitating condition received transplants of neural stem cells directly into their brains. These stem cells have the potential to repair damage caused by MS. After a year of observation, none of the patients experienced serious side effects. Importantly, their clinical condition remained stable, indicating that the disease hadn't progressed further. This was supported by brain scans and lab tests which showed no signs of worsening. While further research is needed, these early findings represent a significant milestone. The safety and potential effectiveness of this stem cell therapy offer hope for a future where progressive MS can be effectively managed, potentially improving the quality of life for countless individuals living with this disease.
